The three rules that decide whether your letter counts

• The termination must reach the insurer before 30 November to take effect the following year.

  "Reach", not "post". The postmark does not count — the day the letter arrives at the insurer does. The FOPH advises sending before 15 November, by registered mail or "A Mail Plus".

• Insured persons may not change insurer if they have not paid everything they owe their fund by 31 December.

  One outstanding premium is enough to void the change. Check your balance before you send the letter, not after.

• All insurers are obliged to accept new members into basic insurance.

  Sign up with the new insurer at the same time as you terminate — do not wait for the old one to confirm.

Those are the FOPH's words. The letter below respects them — but no letter can meet a deadline for you.

How to use it

1. 1Pick your insurer: its official address fills itself in, exactly as it appears in the FOPH register.
2. 2Add your name, address and insured number — they stay in your browser.
3. 3Copy or print the letter, sign it, and send it by registered mail before 15 November.
