Cancellation letter for your health insurer
Fill in three fields and the letter writes itself, with the right address and the required details. Nothing is sent and nothing is stored.
Generate your termination letter
Pick your insurer: the letter is written with its exact address, taken from the official register of authorised insurers.
- 1Pick your current insurerThe letter is filled in with its exact address, taken from the official register of authorised insurers.
- 2Complete and signCopy the text, fill in the square brackets — name, address, policy number — print and sign.
- 3Send by registered mailThe letter must be received by 30 November. Posted late on the 29th, it arrives too late.
- 4Keep the confirmationKeep the receipt and the insurer’s written confirmation: they prove the date of delivery.
Four things not to get wrong
- The letter must be RECEIVED by 30 November — not posted on the 30th. Posted late on the 29th, it arrives on 1 December, and the year is lost.
- Send it by registered mail. Without proof of delivery, a disputed termination is a lost one.
- Your premiums must be paid up. An insurer may refuse the termination while arrears remain.
- Do not terminate your supplementary cover with this letter: different contract, different deadlines — and supplementary cover, once lost, is not always available again.
The three rules that decide whether your letter counts
The termination must reach the insurer before 30 November to take effect the following year.
"Reach", not "post". The postmark does not count — the day the letter arrives at the insurer does. The FOPH advises sending before 15 November, by registered mail or "A Mail Plus".
Insured persons may not change insurer if they have not paid everything they owe their fund by 31 December.
One outstanding premium is enough to void the change. Check your balance before you send the letter, not after.
All insurers are obliged to accept new members into basic insurance.
Sign up with the new insurer at the same time as you terminate — do not wait for the old one to confirm.
Those are the FOPH's words. The letter below respects them — but no letter can meet a deadline for you.
How to use it
- 1Pick your insurer: its official address fills itself in, exactly as it appears in the FOPH register.
- 2Add your name, address and insured number — they stay in your browser.
- 3Copy or print the letter, sign it, and send it by registered mail before 15 November.
