You commit to seeing your designated doctor first, who refers you onward to a specialist if needed. Emergencies stay directly accessible, and most insurers add gynaecology and ophthalmology — but the list of exceptions varies by contract, and it is that list to read before signing.

When it holds up

  • You already have a family doctor and they are on the insurer’s list: the discount then costs you no change of habit.
  • You want someone keeping an overview of your file rather than scattered consultations.
  • You live outside urban areas, where no HMO centre is reachable.

What you accept in return

  • If your doctor retires or leaves the list, it is up to you to find another among those the insurer accepts.
  • Going straight to a specialist without your doctor’s approval can lead to a refusal of cover.
